They 100% send a message. It's pretty nasty too.

So I wonder who can't count. You need to sacrifice 10 friends, but yet 230881 have been sacrificed.

My guess would be that people are defriending some, then finding out about the email that is sent to their friend, so they stop. We did it one night messing around with some friends, just to see what happened. Granted we were all in the room when we did it, we just re-added each other, but there might be people that stop after 1 or 2.

Just post in one of the mafia or mob wars pages on facebook for people to add you and then sacrifice them, worked well for me that way.
