Always found the cover of Maggot Brain by Funkadelic to be unsettling.
There are definitely some albums from The Dwarves that fall into this category. Can't post them on this family friendly site though.
Every used record store I’ve ever been to has at least 5 copies of this.

I was always fascinated by Nazareth's album covers. Hair of the Dog was unsettling.
However Expect No Mercy I thought was perfect.

Pretty much everything from HR Giger is unsettling in some way.
The liner notes and the lenticular version of the CD are pretty "obscure". This is my all time favorite album (peep the avatar), so I don't consider it freaky or creepy, but I certainly understand how people might.Tool - Aenima CD
