Molly Hatchet has perhaps one of the coolest names of all time with badass album art. My dad had these in his vinyl collection. I was excited to hear some METAL. I was rather confused, ha ha!
Hell yeah, Frank Frazetta!

I'm wearing my Eaten Back to Life shirt at the moment.

Check out the first two Waco Jesus album covers. The Destruction of Commercial Scum and Filth. Definitely cannot post here. They are far worse than any Cannibal Corpse art.
Cannibal Corpse immediately came to mind. Some of Dying Fetus' album covers are freaky too.

I love metal art and don't find it creepy, but I can understand how a lot of people would. With that being said, I'll throw a few out there.

Here is my favorite album of all time. Features artwork by serial killer John Wayne Gacy.

Cattle Decapitation and Autopsy have some pretty gnarly album covers...

I used to love Dying Fetus until.....

I went to a party one night and the main guy from Dying Fetus was there. Apparently he flew from Maryland to New Mexico to hang out with a female friend of mine. I met him and he was cool and all. He spent a good portion of the night in the garage jamming on guitar along with my friend Phill, who is probably the best guitarist I have ever known.

Anyways, this dude and Phill started a side band and Phill would email him guitar tracks that he would write and record. I heard that they had almost enough material for an entire album. And then after a couple years, the guy asked Phill if he could use the riffs for a new Dying Fetus album.

I don't know all the details but what I do know is that Phill wrote a majority of the riffs that would appear on the Wrong One to F*ck With album, which many consider to be their best album, and he did not receive any money and did not get his name anywhere on the album. Again, I don't know all the details or what was in the contract, so I'm not sure if the band screwed him over or if it was the record company.

Sorry for getting off topic.

Santana Abraxas was one of the first freaky ones I remember. NSFW (although probably a little late in the thread for that now). My LP had a sticker over the naughty parts.
