I may be incorrect about this, but I seem to recall that a medical hardship application doesn't happen until it's actually time to get that extra season (after eligibility otherwise has run out). I could be completely off-base, though.

I believe that is correct.

That is correct. Technically, the medical hardship can not applied for until after the fourth year of eligibility has been completed. And I do not remember a medical hardship being turned down if the conditions are met.

I don't think there is an appeal process as such for players who request a medical hardship and have never redshirted before. They are automatically granted as long as the player meets the playing time limits (and the school applies). The appeal process only comes into play if the player is asking for a 6th year to complete their 4 years of competition.
