It looked like he kind of slashed through the stomach after he stabbed him. In which case, I think the guts would fall out assuming the stomach cavity was compromised, since nothing would be there to hold them in.I'm not really sure guts would fall out like that after being stabbed.
Depends on the skill of the person doing the cutting, and the location of the cut. The bowel can herniate through a weak point in the abdominal wall...if the skin & abdominal wall were split low on the belly, and the edges were pulled apart, gravity could do the rest. However, it seemed like that cut was a little too high for what was falling out of it.

I know its necessary to break everyone down, especially Rick, and show Negan's power but 8 episodes of it was overkill. We get it. I don't think its a coincidence that ratings are way down - its not really enjoyable to watch your "team" get their asses kicked week in and week out. At least last night's episode was a happy one at the end.
