Core Muscle Injury

It’s not back (I’ve done that).

It’s my abdominals. Lower abs or transverse abs/obliques.
Did the onset of pain happen after any particular activity or form of trauma (doesn’t matter how minor) and was it a sudden onset or did it gradually build.

With the movement of the pain it’s most likely a hernia and not a worse problem in teh testicles. Going in right away is the smart call and after imagine and a HPE they should have you sorted. Hernias don’t always require surgical repair (if that’s what it ends up being) but if your pain level is consistently high that’s most likely going to be the recommended path.

Might I ask why you are seeing ortho? This is more a general surgery consult after going through your PCP or ED

I felt it originally 4 weeks ago working out (all abs at BW)

Then today I noticed it after working out, about 40minutes after I was done. Pain/discomfort has increased over time this AM.

I thought for sure it was core muscle injury. Ortho ER just called and said they don’t see that. Odd thing is there is a sports med doc at KU that is none of core muscle surgery in the Midwest but I can’t get into him until Jan 16.

How do I tell the difference from core injury vs hernia?
Often with a hernia there will be a visible bump vs an abdominal strain where it won’t occur.

Hernia pain often tends to get worse when coughing sneezing or lifting. Stairs like you mentioned are also common causes of pain.

Hernias also tend to have more of a burning pain but if the hernia is strangulated then it can cause very sharp pain in many people.

Gonna ask again, when did you notice the pain and what activity where you doing before noticing?

That’s a pretty common recipe to a hernia especially if you were lifting heavy or doing any power lifting type movements.

I’m not sure what ortho ER is (gonna guess it’s an ortho consult when you went to the emergency room but not positive on that) but whoever it was did you get imaging and did they palpate your abdomen and groin?

Minor discomfort started 4 weeks ago following AB workout.

I lifted heavy today. Coughing, stairs, getting into truck is not fun. I do have a bulge at the exact pain site.

I read testicular pain and bulge is likely when core injury occurs.

“core injury” isn’t really a medical term or thing as a heads up.

It 100% sounds like a hernia of a bulge is present. Needs to get checked out because if it is strangulated (or gets that way) it is a medical emergency. Not saying that’s what you are dealing with but with the severity of pain it should be looked at either by your PCP, urgent or emergency services.

Im just getting on shift so might not be quick to respond for future messages

Vaso reactions while straining are pretty common especially with certain meds, try some more fiber if yo your anyone else has this issue and talk with your PCP
This was years ago when that happened, maybe 15ish. I had actually hurt it in one of those boot camp workout classes and it didn't take a whole lot to escalate from there. Ended up taking a few weeks off from the class and a night with some magnesium citrate to clear up everything else.

My wife just had this last week in clinic. I can't remember what lead up to it but if you are going to pass out anywhere...

I passed out on the toilet once while fighting a stomach bug. Leaned forward to vomit while having diarrhea, passed out, woke up on the floor in a puddle of ****, puke and blood from breaking my nose. 0/10 do not recommend.

Wife said from downstairs it sounded like a bookshelf had fallen over.

Weird that you are feeling a bulge without a hernia especially with a confirmed torn muscle in your abdomen. But if they didn’t tell you the only difference between an abdominal tear and a hernia is the bulge from which an organ is protruding though the tear.

Did you get a general surgery consult or was it just from the ED? Obv practice the rice method in the meantime but it’s always good to ask why surgery is recommended.

Also since you said “not sure exactly” a torn abdominal muscle is a general diagnosis and a torn oblique is a region specific diagnosis. Think of it being similar to someone saying “well I broke my leg” vs “I broke my femur”. Both true but one is more specific
 
Low abs and now my balls hurt. Point tender and can’t hardly do stairs or stand up straight. Ortho ER at 1, can’t wait.

I’ve dislocated both shoulders multiple times, this discomfort is right up there.

Seems kind of strange that a torn oblique would show up as low abdominal/testicular pain. Obliques are along your sides.

I think the takeaway here is to not bother with ab work. Seriously though, getting abs is 95% about reducing bodyfat. They will grow just like any other muscle, but no matter how big they get you have to get to a sub-15% bodyfat before they really show up.

The cremaster muscle (the muscle that causes the testicle to contract upwards) is an extension of the oblique so that’s not unusual to get either direct or referred pain to the region. There is also a ton of inter connectivity between the abdominal muscles and those that extend down into the groin/testicles.

Is it pronounced cream master muscle? That would be pretty accurate if so.
