Considering the old guy in the command center completed nearly this exact same jump ~50 years ago I highly doubt it outside of a huge malfunction.

The difference between this jump and the old guy's is altitude (23 vs 19 miles) and breaking the sound barrier. The balloon (except size) and pod are no different.

They thought they did and said they did in their apology. That's why he was saying "get of it" to the camera. I'm not sure they actually did since he said "get of it" long after the guy shot himself.

After he came back on to apologize, he said there was supposed to be a 5 second delay. Obviously, somebody messed up.
