    yes and unless it happens once then you reboot it and it's fine then it's probably screwed.

    Installing games (for better performance), DLC, etc.. you can burn through HD space pretty quick

    Well on the newest systems it is a red dot instead of the red ring.

    This. Some games actually recommend that you install them for the best performance. They are usually between 5 and 7 GB. You can also download full games from Xbox Live, and they can be even bigger than that. They removed the size limit on downloadable games as well, so it's not uncommon to see them over a GB. Not to mention DLC, demos, vids, and apps.

    I have a 250GB machine and currently have about 100GB free. I try to keep it cleaned up, but it's easy to fill space if you just let it go.
